Primary Care Networks (PCNs)

PCNs were developed in 2019 in response to the PCN DES which is a 5-year contract that the vast majority of practices have signed up for.


Not only are PCNs expected to deliver the requirements of the DES specifications, but they also play an integral role in the development of PCN-based service to patients, working with the community, mental health and acute Providers.


Their role will take on greater significance as the recently published Fuller review is implemented. This gives them an opportunity to review their current models of care and try innovative PCN-level services.
